Our Application packet contains numerous pages of information, along with the application itself. We have included a list of our other senior communities and a checklist to make sure you are returning all pages of the application (please refer to it before you mail/deliver the application).

Please call us at 503-231-4922, if you have any questions on the application process. Our office hours are Monday thru Friday 10:00am to Noon and 1:00pm to 4:00pm.(Closed on Wednesday)

If you plan on mailing your application, our address is: 6404 SE 23rd Avenue, Portland, OR 97202-5434.  If you bring your application into our office it’s best to arrive approximately 30 minutes before the office window closes.  This allows us time to check the application for accuracy and make photo copies of your current valid picture id and social security card; both are part of the application requirements. If you would like a tour, please call ahead to make an appointment.

IMPORTANT NOTE:  It may be that you prefer to only print specific pages, but it is very important that you print the complete application and fill it out before you mail it to us or drop it off at the office.   A completed application is required before a name is added to our wait list.
